ApplyTexas Application for Admission

Dual Credit and Early Admission Application - This form is required each and every semester and must be signed by the student, parent, and principal.

Early Admissions Approval for Sophomores - This form must include the performance indicator and must be signed by the student and principal. 

New Dual Credit Student Packet - This packet includes the required forms and instructions for new dual credit students who are not sophomores.

Returning Dual Credit Student Packet - This packet includes the required forms and instructions for returning dual credit students who are not sophomores.

Sophomore DC Student Packet - This packet is for sophomores only and includes all required forms and instructions.

Authorization to Release Student Information - If this form is submitted in person, it does not have to be notarized.  If the form is mailed or faxed, the form must be notarized.

Certification of Dependency Form is no longer used.  Please use the Authorization to Release Student Information form instead. 

Dual Credit Drop Form - You must visit your high school counselor before dropping.  You will not be dropped until the Admissions and Records Office at South Plains College receives this form.
