Dual Credit/Early Admission Course Approval Form - This form is required each and every semester. It must be signed by the student, parent, and principal.

Dual Credit Student Packet - This packet includes all required forms and instructions to take dual credit.
Authorization to Release Student Information - If this form is submitted in person, it does not have to be notarized. If the form is mailed or faxed, the form must be notarized.
Dual Credit Drop Form - You must visit your high school counselor before dropping. You will not be dropped until the Admissions and Records Office at South Plains College receives this form.
